Brief description

• In the case of the loss, theft, damage, or destruction of a DL or IDL, the holder of a Driving Entitlement(s) must report the fact immediately to the appropriate section of the municipal authority of a municipality with delegated powers, or to the town hall of a city in the Czech Republic, where he/she has his/her permanent residence.

• In the case of theft, the holder shall present to the authorised employee of the appropriate section of the municipal authority of a municipality with delegated powers, or to the town hall of a city in the Czech Republic, an announcement of theft, from the Czech Police department where he/she reported the event.

• Other documents which must be presented, include: an identification document (citizen identification card, passport), in the case of a damaged DL or IDL also that document, and one colour or black-and-white photograph 35x45 mm and a filled-out application form for the issuance of a DL (new model).

• The municipal authority with delegated powers, or the town hall, shall issue a “Confirmation of the Loss, Theft, Damage, or Destruction of a DL or IDL”.  The confirmation is in the form of a Type-6 DL, where in the place of a photograph is a sign stating that this is a “Confirmation of the loss, theft, damage, or destruction of a DL”, and whose reverse side says "valid only for operation in the Czech Republic".

• That confirmation is valid for 30 days, and for that period, it stands in the place of a DL, but only in the Czech Republic.  Once that period passes, the municipal authority of a municipality with delegated powers shall issue a copy of a DL or IDL to the holder of a Driving Entitlement(s).

• The issuance of the “Confirmation of the Loss, Theft, Damage, or Destruction of a DL” is not subject to an administrative fee.

• The administrative fee for issuing a duplicate of a DL or IDL is, the law stipulates, a fee of CZK 50.00, payable in cash.

• If the holder of a Driving Entitlement(s) fails to adhere to the obligations prescribed, he/she may be sanctioned pursuant to Act no. 200/1990 Coll., the Offence Act, as amended.

• The legal process for the processing of the request is regulated by:
Act no. 361/2000 Coll., On Road Traffic and on Changing Certain Acts, as amended, and  by Regulation no. 31/2001 Coll., On Driving Licences and the Driver Registry, as amended.

Immediately upon the notification of the loss, theft, damage, or destruction of a Driving Licence, the municipal authority of a municipality with delegated powers shall issue to the holder of a Driving Entitlement(s) a "Confirmation of the Loss, Theft, Damage, or Destruction of a Driving Licence".
This confirmation is valid for 30 days from its issuance, for which period it stands in the place of a Driving Licence, but only for operation in the Czech Republic.
At the same time, the applicant shall fill out an “Application Form for the Issuance of a Driving Licence”.  Once the confirmation expires, the appropriate municipal authority of a municipality with delegated powers shall issue for the holder of a Driving Entitlement(s) a duplicate of his/her Driving Licence.
