Any natural person can apply for an extract from driver’s registration card related to information about them at any municipal authority with extended powers. List of those (206 in the Czech Republic) is available on the map below. Any natural or legal person can apply for an extract from driver’s registration card of other person only upon a written consent from this person verified by a relevant authority. What to bring with you? Request for an extract from driver’s registration card Valid identity card Written consent from a person, whose information you request, verified by a relevant authority Administrative fee for the extract is CZK 15 for the first page and CZK 5 for any other pages. Deadline is 30 calendar days after submission of the request. Usually, the request is settled immediately.
